Berkeley Optometry Advising Form

If you have further questions about the admissions process or prerequisite courses, you can fill out this form. Advising sessions will be over the phone or Zoom. In person meetings are available upon request.

In order to facilitate a productive session, you may be asked to submit an unofficial copy of your transcripts. You should also provide course descriptions if you have specific questions regarding courses you have taken or are planning to take.

If you intend for us to look at your transcripts, please have it available digitally and be ready to share your screen. We are unable to calculate GPAs. This time will be used to look at academic trend and completed prerequisites. Having your transcripts is not a requirement for our advising session.

Availability

Please input your top three dates and time that you would like to meet with at least 48 hours away from the date of submission. Our standard meeting times are Monday - Friday, 9AM - 4PM PST.
